Voter Registration Form

Need to register to vote? Click for a printable "Application for Voter Registration" http://nashville.gov/vote/docs/Voter_registration_application_revised.pdf. Mail or hand deliver this completed form to your County Election Commission. The address can be found on page 2 of the application.

You may also register to vote at:

  • Your county Election Commission office during regular business hours
  • All motor vehicle driver license stations when applying for a license
  • All offices that provide public assistance (AFDC, food stamps)
  • Agencies that provide assistance to veterans or those with disabilities
  • Offices of county clerks and county registers of deeds
  • You may register on a mail-in registration form. Forms are available from the offices listed above and at all Post Offices.
